Insurance and Payments
We partner with most major PPO insurance carriers. Bring your insurance documents (physical or digital) and a valid ID to your appointment. This helps us verify your coverage and co-pay, allowing us to bill your insurance. In order to confirm that we are a preferred provider in your network, please contact your insurance prior to scheduling your appointment.
Self-Paying Patients
Initial consultation: $350
Follow-up prices vary; payment is due at the time of service. Procedure pricing will be discussed during consultation.
Insurance Coverage
Even with contracted plans or coverage, payment isn’t guaranteed to be fully covered. Insurance companies set pricing for billed services. We do not have access to individual plan details—please verify your coverage beforehand.
